An intrastate truck driver is suing after his insurance company denied coverage.

Piotr W. Wochnik filed a lawsuit on Aug. 29 in Circuit Court of Cook County against Richard Hayes and Integra Benefits Inc., citing breach of fiduciary duty, negligence, detrimental reliance and breach of contract.

According to the complaint, in July 2012, Wochnik applied for health insurance coverage through the defendants after talking to Hayes and Integra Benefits. The plaintiff states he listed his occupation as intrastate truck driver on the forms, was issued a health insurance policy and paid his monthly premiums.

On Nov. 11, 2012, Wochnik claims he was involved in a collision that resulted in medical bills that totaled about $676,849.96. The plaintiff contends one or more of his insurance claims were denied under his policy for various reasons, including his profession.

He alleges the defendants breached their duty by failing to procure an insurance policy that provided adequate coverage for the plaintiff's needs.

Wochnik is seeking reimbursement for all losses, including medical expenses and any other relief. He is being represented in the case by David J. E. Roe.
