A woman is suing a Chicago hospital and doctor over claims their negligence caused the death of her mother.

Dee Adra Simmons, as independent executor of the Estate of Jeannette Simmons, filed a lawsuit Jan. 6 in Cook County Circuit Court against St. Bernard Hospital, Dr. Otuonye Onyewuchi and Doctors Unlimited Service Corp.

According to the complaint, Jeannette was admitted into St. Bernard Hospital on Nov. 5, 2013, due to experiencing decreased responsiveness, altered mental status, fever and poor appetite. Jeannette was eventually assigned to Onyewuchi, the suit states.

Simmons says the defendants negligently removed Jeannette's central line while she was in the upright position and failed to instruct her to hold her breath and perform the Valsalva maneuver, among other alleged acts of negligence.

The defendants' negligence resulted in a decrease in Jeannette's heartbeat rate before she experienced ventricular fibrillation and went into shock, ultimately causing her Nov. 8, 2013 death, the suit asserts.

Simmons is represented by attorney Ryan E. Yagoda of Kralovec, Jambois & Schwartz in Chicago.
