CHICAGO — A family member is suing Chicago health practitioners, alleging negligence led to a relative's death.

Michael Kowalik, independent administrator of the estate of Jacob Kowalik, filed a lawsuit April 13 in Cook County Circuit Court against The Children's Memorial Hospital, now known as Ann & Robert H. Lurie Children's Hospital of Chicago, Children's Hospital of Chicago Faculty Practice Plan Inc., Pediatric Faculty Foundation, Inc., and Dr. Reggie E. Duerst, alleging negligence in wrongfully replacing Jacob Kowalik's dosage.

According to the complaint, on Oct. 15, 2009, Jacob Kowalik was admitted to Chicago Children's Hospital for a stem cell transplant. The suit says his condition began to worsen on Nov. 6, 2009, when his dosage of cyclosporin was changed from intravenous to oral administration. 

The complaint states Kowalik's condition gradually weakened, leading to his death April 15, 2014. The plaintiff alleges the defendants failed to properly convert plaintiff's medication dosage, which caused his death. 

Michael Kowalik seeks damages from each defendant of more than $50,000. He is represented by attorney John M. Saletta of Law Offices of John M. Saletta PC in Chicago.
