A woman is suing a nursing home on behalf of her elderly father, alleging neglect and wrongful death.

Katherine Forman, administrator of the estate of Kenneth Twadell, filed a lawsuit Sept. 28 in Cook County Circuit Court against Advocate Health and Hospitals Corp., doing business as Advocate Christ Hospital and Medical Center; Manor Care of Oaklawn (East) Illinois, doing business as Manorcare Health Services-Oak Lawn East; Manor Care Health Services, doing business as HCR Manor Care Health Services, and Heartland Employment Services.

According to the complaint, Twadell was admitted to Advocate Christ Hospital on Oct. 20, 2013, with a urinary tract infection and left-side weakness. He was discharged on Oct. 24, 2013, to Manorcare Health Services-Oak Lawn East. During his stay at the care facility, he allegedly developed several worsening pressure sores that became infected and required debridements and hospitalizations. The suit claims the defendants’ negligence led to his injuries and death on Dec. 5, 2013.

Forman is seeking reimbursement for medical, hospital and funeral expenses totaling more than $50,000. She is represented by attorneys Jordan S. Powell and Mary K. Volk of Levin & Perconti in Chicago.

Cook County Circuit Court case number 2015L009854.
