CHICAGO — A Cook County woman is suing Lakeview Rehab, a skilled nursing facility, claiming her father died there in 2014 from injuries he suffered due to alleged negligence.

Lisa Shaffer, independent administrator of the estate of Richard Berry, filed a lawsuit Jan. 26 in Cook County Circuit Court against Lakeview Rehab & Nursing Center, Infinity Healthcare Management of Illinois LLC, registered nurse Sherwin Basillo, and licensed practical nurses Irene Mendoza and Cierra Aaron, alleging violations of the Nursing Home Care Act and wrongful death.

According to the complaint, Berry was admitted to Lakeview on June 8, 2012, for ongoing rehabilitation and was suffering from dementia. While living at Lakeview, the suit claims, Berry developed several conditions, including muscle or tendon shortening, as well as skin breakdown and pressure ulcers on various areas.

After Berry was found unresponsive on Jan. 28, 2014, the suit alleges, he was transferred to an acute care facility where he was diagnosed with dehydration, kidney damage, urinary tract infection, a heart attack, infected pressure wounds and other conditions. Once he was sent back to Lakeview on Feb. 8, 2014, the lawsuit claims, he developed additional skin breakdowns and pressure ulcers with necrosis, passing away from his injuries on Feb. 28, 2014.

Shaffer seeks compensation for injuries, losses and damages, in an amount to be determined at trial. She is represented by attorney Jacqueline Kanter of Kralovec, Jambois & Schwartz in Chicago.
