A woman is suing a doctor, health center and pair of pharmacies over claims their alleged mistakes led to her mother's death.

Brenda Bridgeforth, as special administrator of the Estate of Audrey Bridgeforth, filed a lawsuit March 28 in the Cook County Circuit Court against Dr. Gabrielle Mykonutis, Advocate Health Centers Inc., American Drug Stores L.L.C.,and Osco Drug No. 3092, citing medical malpractice and wrongful death.

According to the complaint, Mykonutis, a doctor at Advocate, changed Audrey's medications in March 2012 while treating her for high blood pressure. Bridgeforth claims her mother suffered an intracranial bleed on March 29, 2012, which led to her death the next day.

Bridgeforth further alleges that a drug store owned by American Drug Stores and Osco Drug on East 95th Street in Chicago failed to properly fill her mother's prescription or incorrectly marked the dosage.

She is seeking more than $50,000 in damages and is being represented in the case by attorney Charles V. Falkenberg III of Karlin, Fleisher & Falkenberg LLC.
