A Cook County man is suing over claims of the Wrongful Death Act against a certain physician and physicians’ group of an Illinois-based health care center.

James Lewis, as special administrator of the estate of Cindy Lewis (deceased), filed a lawsuit Aug. 18 in Cook County Circuit Court, Law Division against Deepti Agarwal, M.D., in conjunction with Bishan Chakravarty, M.D., Chicago Anesthesia Associates SC and Advocate North Side Health Network, doing business as Advocate Illinois Masonic Medical Center, citing wrongful death.

According to the claim, Cindy Lewis suffered an untimely death while in the care of Agarwal and the aforementioned doctors and organizations when she became unresponsive after her blood oxygen saturation level decreased while under an anesthesia during a left trans-pars plana vitrectomy procedure at Advocate Illinois Masonic Medical Center on Jan. 5.

James Lewis is seeking damages of more than $50,000, plus the request of a jury, and court costs of this litigation for Agarwal and the aforementioned physicians’ and organizations’ negligence to not properly treat and care for Cindy Lewis, the claim states.

He is being represented by Robert B. Phillips and Associates LLC in Chicago.

Cook County Circuit Court, Law Division case number 2015L008423.
