A widower has filed a wrongful death claim against two health care facilities on behalf of his wife’s estate.
Edward A. Mack, also known as E. Terry Mack, filed a lawsuit Sept. 25 in Cook County Circuit Court against NorthShore University HealthSystem, doing business as Glenbrook Hospital; NorthShore University HealthSystem Faculty Practice Associates; Noah DeGarmo M.D.; Kelly Izsak P.A.-C; and Sandra Leeson P.A.-C. Mack is independent executor of the estate of Barbara Mack, his wife.
According to the claim, Barbara Mack was admitted to Glenbrook Hospital on Sept. 29, 2013, where she was allegedly treated by DeGarmo, Izsak and Leeson. The suit says the defendants failed to timely diagnose and properly treat Barbara Mack’s subdural hematoma and cranial bleeding. As a result, the suit claims that Mack sustained serious and permanent injuries that allegedly led to her premature death on April 9, 2014.
Edward Mack is seeking damages of more than $50,000. Attorneys Amanda L. Brasfield and Craig P. Mannarino of the Chicago firm Kralovic, Jambois & Schwartz are representing the plaintiff.
