Cook County woman alleges abuse, neglect by nursing home

A Cook County woman is suing a nursing home, alleging medical malpractice in the death of a relative.

Jacqualynn Deanes, independent administrator of the estate of Ophelia Swindle, filed a lawsuit Sept. 29 in Cook County Circuit Court against Park Villa Nursing and Rehabilitation Center, Chaim Rajchenbach, Menachem Berger and Menachem Shabat. Named as respondents in discovery are Christopher Kropp, Dr. Mohamad Zakkar, Dr. Asad Zaman, registered nurse Jamie Doljanin, nurse Christina Galvin, nurse Eileen Walczak, Bernadette Grogan and Jeffrey Floyd.

According to the complaint, Ophelia Swindle was a resident of Park Villa Nursing and Rehabilitation in Palos Heights from Oct. 18-25, 2013. The suit says while Swindle was in the care of Park Villa she developed worsening complications related to a gastronomy tube, was given psychotropic drugs without her guardian’s consent, and otherwise suffered from abuse and neglect resulting in injuries that allegedly caused her death Oct. 28, 2013.

Deanes seeks more than $50,000 in damages, plus litigation costs. She is represented by attorney Stephen S. Phalen of the Law Offices of Stephen S. Phalen in Chicago.
