Wrongful death suit accuses Oak Park HealthCare of neglect in elderly woman’s death

Head injury

A Cook County woman is suing her mother’s nursing home and care givers, alleging negligence in failing to prevent a fall that led to her mother's death.

Hattie Jones, independent administrator of the estate of Ada Hannah, filed a lawsuit Aug. 27 in Cook County Circuit Court against Oak Park HealthCare Center, Paramount of Oak Park Rehabilitation & Nursing Center, Care Plus Management Inc., Extended Care Consulting, Extended Care Clinical, Clarence Boykin, Richard Taylor, Helen Lacek, David Wengrow, David Porush and Dr. Richard Risner.

According to the complaint, Hannah became an Oak Park HealthCare resident after Aug. 1, 2013, requiring assistance with daily activities and supervision to avoid falls. On at least four occasions, the suit says, Hannah fell and suffered head injuries while under the defendants' care.

On Oct. 14, 2013, the lawsuit states, Hannah suffered a fall that sent her to the emergency room with another subdural hematoma in addition to one he already had, which caused her death Oct. 26, 2013.

Jones seeks damages of more than $50,000, plus legal costs. She is represented by attorney Joseph R. Curcio of the Curcio Law Offices in Chicago.

Cook County Circuit Court case number 2015-L-008764.
