CHICAGO — A former patient is suing Momence Meadows Nursing and Rehabilitation Center, citing alleged negligence that resulted in an injury.

Katrina White filed a complaint on March 8 in the Cook Circuit Court against Momence Meadows Nursing and Rehabilitation Center LLC and Infinity Healthcare Management of Illinois LLC, alleging that they knowingly violated the Illinois Nursing Home Care Act by failing to provide proper assistance and supervision in the care and treatment services.

According to the complaint, the plaintiff alleges that, on April 5, 2014, she suffered an unwitnessed fall resulting in a fracture in her ankle. The plaintiff holds Momence Meadows Nursing and Rehabilitation Center and Infinity Healthcare Mangement of Illinois responsible because the defendant allegedly was responsible for properly assisting, monitoring and supervising her condition to prevent her from falling.

The plaintiff seeks compensation for an amount in excess of $50,000, plus attorney's fees and the costs of the suit. She is represented by Michael L. Shanahan of Levin & Perconti in Chicago.
