CHICAGO — A woman is suing Windmill Nursing Pavilion, Dynamic Healthcare Consultants and a nurse for allegedly taking insufficient measures to prevent injuries and wrongful death.

Cheryl Sims, independent administrator of the estate of Theodore Harvey, filed a complaint on Dec. 1 in Cook County Circuit Court, alleging Harvey suffered a fall that allegedly caused an acute ankle fracture, an oblique fracture of the distal fibula and an avulsion fracture. The plaintiff holds the defendants responsible for allegedly failing to adequately assist Harvey to address his risk of falling.

The plaintiff seeks judgment against the defendants in an amount greater than $50,000. She is represented by Jaime Koziol Delaney and Nadia Adam Ahmed of Levin & Perconti in Chicago.
