CHICAGO — A man is suing Westchester Health and Rehabilitation Center, citing alleged negligence and wrongful death in the care of a relative.

Raymond Las Cola filed a complaint on July 30 in Cook County Circuit Court against Westchester Health and Rehabilitation Center alleging Mary Las Cola, who was then a resident of defendant's nursing care facility located in Westchester, suffered from weight loss, dehydration, malnutrition and deterioration of her physical and mental health prior to her death on in December 2016, as a result of the negligence of employees of the defendant.

The plaintiff alleges Westchester Health and Rehab failed to implement adequate resident care plans based on comprehensive assessment and individual needs of its residents, failed to notify resident's family for changes in her condition, and failed to adequately train nursing staff and aides in the procedure for supervising residents.

The plaintiff requests a trial by jury and seeks judgment against defendant of more than $50,000, plus attorney's fees and costs. He is represented by Kristen N. Koeller of The Law Offices of Steven J. Malman & Associates PC in Chicago.
