CHICAGO — Glenwood Healthcare & Rehab Inc., Glenwood Terrace and Certified Health Management Inc. are accused of violating the OBRA/Nursing Home Care Act in a lawsuit involving a nursing home resident who allegedly suffered several falls and was injured.

Denette Borum, an independent administrator, filed a complaint on Sept. 12 in Cook County Circuit Court, alleging between March 26, 2017, and Feb. 1, 2018, Alam was a resident of Glenwood Healthcare & Rehab, where she allegedly had several falls that resulted in injuries. The alleged injuries included a fractured left hand, head injury and trauma, bleeding from the brain, acute cervical strain and and rib fractures. Alam allegedly required surgery.

The plaintiff alleges the defendants knew of Alam's medical conditions, including blindness, but did not offer assistance at the level she allegedly required to perform activities of daily living.

The plaintiff requests a trial by jury and seeks an amount greater than $50,000, plus attorney fees and costs. She is represented by attorney James C. Ferrell, of Houston.
