Countryside Nursing and Rehab Center faces wrongful death suit from daughter of former resident

CHICAGO – The daughter of a former resident of a Dolton long-term nursing care facility alleges it failed to provide properly supervised care.

Jerena Hawthorne-Jones, as independent administrator of Carolyn Hawthorne, filed a complaint on Oct. 22 in Cook County Circuit Court against Countryside Nursing and Rehab Center, alleging wrongful death.

According to the complaint, the plaintiff alleges between October 2014 and Feb. 28, 2017, Hawthorne-Jones was a resident at Countryside Nursing and Rehab Center. The suit states Hawthorne-Jones developed multiple pressure ulcers that worsened in condition and became infected, causing a deterioration in her physical and mental condition. The suit states she died in February 2017.

The plaintiff alleges the defendant failed to provide properly supervised nursing and personal care to Hawthorne-Jones and failed to adequately asses her for her risk of the development of pressure ulcers.

The plaintiff requests a trial by jury and seeks compensation of more than $50,000. She is represented by Jason M. Kellerman of The Law Offices of Steven J. Malman & Associates PC in Chicago.
