CHICAGO — A woman is suing Wentworth Rehabilitation and Health Care Center Inc. and Alden Management Services, long-term care facility, citing alleged negligence.

Diann Baker filed a complaint on Jan. 9 in Cook County Circuit Court alleging, in September 2017, Andrew Williams Jr., a resident in the defendants' health care center, suffered a fall, resulting in a fracture to his right hip requiring surgery. The lawsuit alleges the fall and injuries caused pain and suffering, disability, disfigurement, and medical and related expenses that caused and contributed to his death in April 2018. Additionally, the Williams' next of kin suffered the loss of companionship and society, grief, sorrow and mental suffering.

The plaintiff alleges the defendants failed to protect Williams from neglect, failed to implement appropriate interventions to prevent falls, failed to properly train individuals who provided care for him, and failed to timely notify his physician and family regarding changes in his condition.

The plaintiff requests a trial by jury and seeks more than $50,000. She is represented by Michael Shanahan and David Bulgarelli of Levin & Perconti in Chicago.

Cook County Circuit Court Case No. 19-L-279.
