Ray Graham Association, of Addison, named in lawsuit after man dies choking on banana bread

CHICAGO — A man is suing Ray Graham Association For People with Disabilities, in the choking death of a man who was a resident at the Ray Graham facility in Addison.

Brian Nach filed a complaint on Jan. 16 in Cook County Circuit Court.

According to the complaint, the plaintiff alleges in September 2018, Jeffrey Nach was a resident of defendants' living facility. At that time, the complaint alleges he choked on a banana bread, developed an obstructed airway and eventually suffered a cardiac arrest and died. The plaintiff alleges the negligence of employees of the defendant in providing medical and personal care led to the incident and Nach's death. 

The plaintiff alleges the defendants failed to follow Nach's individual service plan for eating, failed to ensure he was properly supervised, and failed to properly train staff to identify, observe and prevent the choking hazard.

The plaintiff requests a trial by jury and seeks more than $50,000, plus attorney fees and costs. He is represented by Margaret P. Battersby Black and Michael L. Shanahan of Levin & Perconti in Chicago.
