Daughter blames Glen Oaks Nursing and Rehabilitation for mother’s injuries, death

CHICAGO – A Cook County woman is suing a Northbrook health care center, alleging negligence in her mother’s death.

Tessan Johnson, independent administrator of the estate of Vivian Johnson, filed a lawsuit Dec. 4 in Cook County Circuit Court against Glen Oaks Nursing and Rehabilitation Centre and Glen Health and Home Management Inc., alleging violations of the Illinois Nursing Home Care Act.

According to the complaint, Vivian Johnson, 55, was a resident of Glen Oaks from June 19, 2013 to June 5, 2015, with intermittent hospitalizations. Requiring care with daily living activities, including ambulation and transfers, Johnson was at risk for falls, the suit says.

After a Dec. 12, 2013 fall, the plaintiff alleges Johnson suffered a right humerus fracture and a subdural hematoma, leading to her death June 16, 2015.

Tessan Johnson seeks damages of more than $50,000, plus litigation costs. She is represented by attorney Bradley S. Thomas of Levin & Perconti in Chicago.
