CHICAGO — A man is suing dentist James V. D'Alise and A Center for Dental Implants for allegedly failing to comply with his records request.
David Houghton filed a complaint on Aug. 31 in Cook County Circuit Court, alleging the defendants failed to provide the records he allegedly requested, as he also is accusing the defendants, among other things, of negligently treating his periodontal disease.
The plaintiff seeks judgment against the defendants in an amount greater than $50,000 plus court costs. He is represented by Anthony E. Blumberg & Associates LLC in Chicago.
