Symphony of Chicago assisted living facility accused of negligence in patient's death

CHICAGO — An estate administrator is suing an assisted living facility, alleging that an elderly man died while living there due to the staff's negligence.

Rita Welch, as independent administrator of the estate of decedent Larry Crowder, filed a complaint on March 27 in the Cook County Circuit Court against Symphony Jackson Square LLC, dba Symphony of Chicago West, et al. 

According to the complaint, Larry Crowder was a resident of the defendant's facility at 5130 West Jackson Blvd. Crowder allegedly was caused to contract pressure injuries and, as a result, died while under the care of the defendant who was negligent in their care for the plaintiff.

The defendant is accused of wrongful death and negligence.

The plaintiff is seeking all reasonable sums due, attorney fees and court costs. The plaintiff is represented by Colleen Mixan Mikaitis and David Bulgarelli of Levin & Perconti in Chicago.

The Cook County Circuit Court Case No. is 2019-L-003303.
