A Cook County woman is seeking restitution from Hexagram Home Health Care for injuries allegedly suffered in a fall.

Johnnie Blueitt of Chicago filed suit on Sept. 4 in Cook County Circuit Court against Hexagram Home Health Care LLC, doing business as Hexagram Home Health Care and located in Chicago.

According to the complaint, Blueitt was receiving respite care and/or treatment in her home from Hexagram on Oct. 31, 2013, when she allegedly fell while being transferred from one place to another. This fall allegedly resulted in various injuries, including but not limited to a right femur fracture that required surgery.

As the plaintiff had known mobility issues, the defendants were allegedly well aware that she was at risk for falls and should have been prepared to take extra caution in matters involving her being transferred from one place to another, the complaint states.

As such, the plaintiff seeks restitution for the failure to properly and adequately assess the plaintiff as a high risk for falls; failure to adequately train their staff; failure to adequate reassess the plaintiff's ongoing condition; failure to provide adequate assistance; and other general negligence, as well as restitution for pain and suffering, disability and disfigurement, and medical and related expenses, including attorney fees and court costs.

The plaintiff seeks an amount of more than $50,000. She is being represented by Marc S. Weissman of the Law Offices of Steve J. Malman & Associates P.C. of Chicago.

Cook County Circuit Court case number is 2015L009154.
